IOL이 어떻게 될 것인가? 아니, 무엇보다도 뭔가 잘못되었을 때 환자의 시력에 어떤 영향이 발생할 것인가. Maxwell 박사는 이러한 사태에 대비, 문제가발생할경우IOL기능중원거리시력만 최적화되며 기본 구성인 단초점 IOL이 된다고 설명했다. “이렇게 되면 환자는 독서용 안경을 착용해야 합니다.” 부주의한 환자는 이를테면 장기 휴가 중 전지를 충전해야 한다는 사실을 망각할 수도 있다. 회사는 이런 경우를 대비해 장기 동면 방식(hibernation mode)을 마련해 두고 있다. 제품이 충전되지 않으면 기본 구성인 단초점 수정체로 복귀하며 최장 9개월 후에 재부팅할 수 있다. 이뿐이 아니다. 이 제품은 모든 기능을 프로그래밍할 수 있으며 또한 사용 환경에 적합하도록 설정을 변경할 수 있다. 즉, 담당 의는 특정 환자의 요구를 고려, IOL 추가 전력 스위칭 포인트의 민감도와 배율을 최대 3/4 D까지 원격 조정할 수 있다. Mazzocchi는 다음과 같이 말한다. “이식 가능한 의료 기기 중 이처럼 정교한 컴퓨터 칩과 알고리즘을 사용한 경우는 없었습니다. 우리 제품은 작동한 지 300초 이내에 환자 고유의 동공 동역학을 습득하고 스스로 알고리즘을 조정합니다. 시간이 흐르면 환자의 요구 조건이 변화하므로 담당의는 환자 내원시 이 알고리즘을 리부팅하고 프로그램을 원격으로, 그리고 비침습적으로 수정할 수 있습니다. 각 환자에게 특화돼 있고 적응형으로 작동하며 프로그래밍 가능한 IOL입니다.” 극복해야 할 문제 Lingering questions 이렇듯 ELENZA가 자랑하는 많은 장점에도 불구하고 안전성, 기술 등 제품을 시장에 출시하기 전 회사가 반드시 해결해야 할 문제가 남아 있다. 일례로, 수정체가 YAG 레이저에 조사될 경우 전자 부품에 발생할 수 있는 현상, 사용된 재료의 독성 여부, 제품의 누출 문제 등을 꼽을 수 있다. “전지는 사파이어로 코팅한 후 순금 포장 으로 밀봉된 상태입니다. 순금 포장재와 밀폐된 전지를 시험, 건전성을 확인했습니다. 모든 전자적 구성품은 얇은 유리 웨이퍼 (실리콘박편)에 넣어 밀폐한 후 기존의 단초점 IOL 내부에 장착했는데 이 역시 건전성을 시험, 확인했습니다.” Mazzocchi의 설명이다. 수용해 왔습니다. 특별한 합병증이 관찰되지 않는 한, 특히 고대하던 목적을 달성하는데 도움이 되는 신기술은 언제나 잘 받아들 였습니다. 우리는 첨단 기술을 거부감 없이 잘 수용하는 사람들입니다.” EWAP Editors’ note: Drs. Lindstrom and Maxwell 박사는 “현재로서는 화학적, 공학적 수준에서는 수정체 재료에 독성이 없는 것으로 판단하고 있습니다. 그러나 시험 해 보기 전까지는 어떤 것도 알 수 없습니다.” 라고 말했다. 기존과 같은 작은 절개를 통해 수정체를 삽입할 때 난시가 발생하지 않게 하는 것 역시 해결해야 할 문제다. “우리는 IOL이 접힐떄 주름이 발생해도 모든 내부 전자 구성품의 건전성이 손상되지 않도록 제품을 설계했습니다. 또한 IOL이 절개에 꼭 들어맞을 수 있도록 하는 주입 장치를 설계, 제품이 작은 절개를 통해 삽입 돼도 외상이 발생하지 않도록 했습니다.”라고 Maxwell 박사는 설명을 계속했다. 2014, 2018년 출시 예정 Look for it in 2014, 2018 ELENZA는 2012년 한 해를 상기한 사례 및 기타 우려를 하나씩 없애가는 데 보내고 있다. 이 결과, 최종 완성된 임상 등급 제품 출시가 임박한 것으로 보고 있다. 무엇보다 인체를 대상으로 하는 시험을 오는 2013년 유럽에서 시작할 예정이다. Lindstrom 박사는 “ELENZA는 2014년 초 CE 마크를 획득할 것으로 기대하고 있습 니다. 제품을 인체에 처음 삽입하는 시험을 시작한 후 4-5년이면 FDA 인증을 취득할 수 있을 것으로 봅니다.”라며 “2018년에는 미국 땅에서도 이 제품을 볼 수 있다는 뜻입니다.” 라고 덧붙였다. “바로 이 순간, 나에게 ELENZA는 정말 멋진 계획입니다. 계획을 추진하는 동안 예상치 못한 문제가 항상 발생했지만 우리는 반드시 성공할 수 있다고 굳게 확신합니다.” 제품이 미국에서 시판되려면 수 년을 더 기다려야 하지만 Mazzocchi 대표 이사와 Maxwell 박사는 의사와 환자 모두 컴퓨터 칩과 전지를 눈에 삽입한다는 사실에 거부감을 가지지 않을 것으로 믿고 있다.
