It is surprising how a small fragment of nucleus in the angle can result in extensive endothelial loss over a few weeks. Early removal within the first few days can prevent endothelial decompensation and underscores the importance of surgeon review in the immediate postoperative period. TASS can also present as unexplained corneal oedema postoperatively, and its treatment should be limited to medications with proven benefit or those required in specific situations. Although endothelial cell counts pre- and post-op are often not recorded, I routinely perform this in my practice. Monitoring endothelial cell loss is a useful parameter for benchmarking surgical technique. The mean endothelial cell loss, expressed as a percentage in the last 435 eyes I operated on, was 5.4%—which falls at the lower end of the expected range for modern phacoemulsification.
