30 EWAP FEATURE December 2017 Preference and conviction For standard cases, Dr. Chee said, the visual outcomes are not dif- ferent between phaco and femto. Femto is useful in cases with low endothelial cell count, capsular fibrosis, big eyes in which manual capsulotomies cannot be precisely sized, cataracts with a thick hard nucleus, eyes shallow anterior chambers, subluxated lenses, and in creating arcuate incisions. It should be avoided in cases of eyes difficult to dock, advanced glaucoma, small pupils, significant corneal opac- ity, large pterygium, significant conjunctival chalasis, uncoopera- tive patients, and where cost is a concern. Ultimately, she said, it depends on the surgeon’s preference and conviction. EWAP References 1.
